Just to add a note to Brent's post, if you send a letter outlining what's happened directly to the doctor, all you have to do is mark the envelope "PRIVATE AND CONFIDENTIAL". Support staff are, generally, not supposed to open any letter so marked.

I would strongly urge you to make your displeasure known directly to the doctor in question, as well as to your GP (copy your GP on the letter even).
